Urethroplasty is a surgical intervention aimed at restoring urodynamics and patency of the urethra.
Urethroplasty is a surgical procedure aimed at restoring urodynamics and patency of the urethra.

General information

The urethra is the final part of the urinary tract. In addition to the main function of removing urine from the bladder, it serves as a channel for ejaculation of sperm. There are a huge number of urethroplasty methods for both urethral strictures and developmental anomalies.

The choice of plastic surgery in the urinary tract depends on the location and nature of the pathology, as well as on the qualification level of the surgeon.

Procedure

Preparation

Before the operation, the urologist evaluates the condition of the urethra using an X-ray and tests. This helps the surgeon determine the condition of the urethra and how the surgery should be performed.

The process of urethral plastic surgery

Urethral plastic surgery is performed as follows:

  • The patient is given anesthesia and the surgery is performed under general anesthesia.
  • The patient is placed on a chair that resembles a gynecological chair. The legs should be wide apart to allow direct access to the working area.
  • A marking is made around the scrotum, which goes to the border with the gluteal fossa.
  • Wetting the area with a swab, the surgeon makes an incision.
  • To expose the urethra for further work of the surgeon, the open tissues are spread apart, and then sutured to the skin on the right and left sides with one suture-grasp.
  • A part of the urethra is covered with a sterile tourniquet and lifted up, thus separating it from the surrounding tissues.
  • Using a clamp, the doctor squeezes the urethra and cuts off the deformed part of the canal.
  • The surgeon measures the length of the part that was excised using a metal tube with a closed end.
  • At the upper part of the urethra, the doctor makes a small incision.
  • A plastic tube is used, into which the liquid flows.
  • The surgeon sutures the upper edges of the urethra with catgut threads.
  • The plastic surgery is performed using the "end to end" method.
  • A plastic tube remains inside the urethra, which is inserted through a puncture under the scrotum during surgery. This catheter remains for some time after the operation.

Indications and contraindications

Urethroplasty has the following indications:

  • urethral stricture;
  • penile fistulas;
  • urinary tract malformations;
  • urethral rupture.

If we are talking about a urethral rupture, then about 5 months should pass after the traumatic injury before urethroplasty is performed. Then a scar will be completely formed in the connective tissue in the damaged area.

Contraindications

Urethroplasty has its own contraindications:

  • unstable and severe condition of the patient;
  • presence of urinary leaks;
  • diastasis of the ends of the urethra;
  • severe damage to the urethra;
  • more than 12 hours have passed since the injury.

Complications

Urethroplasty has a number of complications that can appear in isolation or in combination with each other:

  • recurrence of stricture;
  • meatostenosis;
  • narrowing anastomosis;
  • urinary incontinence;
  • diverticulum;
  • erectile dysfunction;
  • penile deformity.
