General information

In order for the operation to be successful and the wounds to heal quickly, during the operation and during further rehabilitation for a short time, urine is removed from the bladder using catheters.

Resection can be performed using several types of surgical interventions:

  • Marion-Holtzoff operation;
  • Solovov P.D. operation;
  • Bude operation;

Before the operation, the patient must take a supine position with the thighs raised to the body. In men, the scrotum and penis are raised as much as possible and fixed. Then, using a special cutter, the narrowed area of the urethra is opened. Then, the back wall of the urethra is separated from the narrowed segment. The narrowed segment is carefully pulled closer and a provisional suture is applied at the sites (slightly below and slightly above the future incision). Then, the narrowed area is cut with a longitudinal incision. Then, both ends of the cut urethra are sutured along the entire circumference, starting from the back wall, with a catgut thread. After this, the healthy tissues above the urethra are sutured.

A distinction is made between segmental (or partial) and complete resection of the urethra. Complete resection is used only if a tumor is detected.

Solovov's Method

This type of operation is recommended for persistent narrowing of the segmental part of the urethra. In the same position, a special metal bougie is inserted into the patient's urethra. Next, as in the Marion-Holtz operation, a resection is made and the bulbous part of the urethra is exposed. A transverse incision is made at the narrowing, and the ends of the urethra are attached to a metal bougie. Next, a series of manipulations are performed using silk ligatures, an antegrade metal bougie, and a suprapubic fistula. After all manipulations are completed, catgut sutures are applied to the wound, and a catheter is removed from the bladder.

The Budé Method.

The Budé operation is used in cases where it is impossible to narrow the resected ends of the urethra and suture them. In this case, the same manipulations are performed to resect the urethra, and the missing section is made from the skin of the scrotum. For this, a rectangular flap of skin is cut from the scrotum to the size of the missing section of the urethra. This flap of skin is rolled into a tube and sutured. Then it is inserted into the space between the cut ends of the urethra and sewn. After the entire procedure, the wound on the scrotum and perineum is sutured.

Rehabilitation course

The rehabilitation course after urethral resection depends on the complexity of the operation. As a rule, it can last from a week or longer.

Indications

Resection of the urethra is performed in the following cases:

  • single narrowing of individual segments of the urethra;

Complications

During the operation to remove the urethra, the following complications may occur:

  • damage to the urethra;
  • "piercing" the urethra or capsule of the prostate, bladder, abdominal cavity of the rectum;
  • erection of the penis.

After the operation, some patients experience early postoperative bleeding, urinary incontinence. There is also a risk of infection.
