General information

Urethral dilation is usually performed to restore normal urination of the patient. It is important to say that this procedure often gives only a temporary result, so it must be repeated periodically. Bougienage is performed using a special metal rod (bougie).

Procedure

Preparation

Before starting the procedure, in addition to the previously conducted diagnostic studies that showed urethral stricture and the number of cicatricial stenosis, the doctor must perform the so-called calibration of the urethra. This is done to determine the diameter of the narrowed part of the urethra and select a bougie of the required size. Then the patient is washed, the head of the penis and the external opening of the urethra are treated with tampons specially designed for this purpose (for men).

The specificity of performing urethral bougienage is the expansion and stretching of the place of the urethra in which a narrowing has occurred.

The process of urethral bougienage

This procedure can be carried out in two ways: by inserting a special bougie or a special balloon catheter into the urethra and is similar to the process of inserting a metal catheter into the urethra.

Using the bougie:

  • The patient lies on his back in the urological chair.
  • The end of the bougie is lubricated with a special gel.
  • A bougie with an oval tip for women and a filiform bougie for men is inserted through the urethra, following through all its sections all the way to the bladder cavity. The instrument is in the canal for 5-10 minutes, after which it is removed. The bougies are inserted sequentially (from the smallest diameter to the largest), until there are no difficulties with removing the instrument. Then the urethra is washed with an antiseptic.

Using a catheter:

  • The patient lies on his back in the urological chair.
  • The end of the catheter is also lubricated with a special gel.
  • The catheter is inserted into the urethra so that the balloon is located at the site of narrowing of the urethra. Then sterile water is introduced into the balloon, after which it expands.

Rehabilitation period

To avoid any complications of an inflammatory nature that arise as a result of urethral bougienage, the patient is recommended to take antibiotics.

Indications

Urethral bougienage is performed in the following cases:

  • short urethral strictures;
  • long urethral strictures, but with uniform narrowing (5-6 cm long);
  • pathological changes in other parts of the urinary system, usually located higher.

Contraindications

Urethral bougienage is contraindicated in case of:

  • inflammatory diseases of the urinary system (cystitis, prostatitis, pyelonephritis);
  • post-traumatic condition of the urethra.

Complications

The risk of possible complications during this procedure, unfortunately, is very high. Among them are the following:

  • urethral fever;
  • inflammatory processes in the genitals;
  • significant damage to the urethra.
