The process of the operation

Preparation

Preparation for surgical intervention consists of the following sequential stages:

  • general and biochemical blood tests, which determine the amount of protein, glucose, leukocytes, creatinine, urea and bilirubin;
  • test for hepatitis C and B, as well as blood clotting;
  • x-ray of the affected organ;
  • restriction in food intake one day before the operation;
  • shaving hair in the groin area;
  • two days before the operation, it is necessary to stop taking blood thinners;
  • patient examination by an anesthesiologist

The process of performing surgery on the urethra

There are several types of this surgical intervention:

  • Bugienage. This operation is performed using a special bougie instrument under general anesthesia. At the beginning of the operation, a catheter is inserted to drain urine from the bladder. Then a balloon or regular bougie is inserted into the narrowed area, and upon visual contact the narrowed area is expanded. This operation is used to treat urethral strictures.
  • Urethrotomy. This operation also begins with anesthesia and insertion of a catheter. Then a special device (urethrotome) is inserted through the urethra directly to the stricture or tumor. When the urethrotome reaches the stricture, a thin blade is pulled out of it and the stricture is dissected with it. Next, a rubber catheter is inserted into the dissected area so that the dissected edges of the stricture do not grow together. In about 5 days, the wound heals and the catheter is removed.
  • Meatotomy. When treating narrowing of the urethral canal using this surgical method, the stricture is dissected in the narrowed area, and then sutures are placed on the dissected edges. This method allows the wound to heal. In this case, no scar will form that could recur into a new stricture.
  • Urethroplasty. This is a plastic surgery to eliminate strictures in the urethra. This operation on the urethra is used when the stricture is widespread. In this case, the narrowed ends are dissected and the urethral stricture section is removed. Then, to make an implant, a flap of skin is taken from the scrotum or penis according to the size of the removed section. The flap of skin is folded into a tube and the edges are sutured to the cut edges of the urethra.

Rehabilitation course

After the operation, the patient is prescribed antibacterial drugs. During the first three days, a bandage is changed daily. The catheter will be removed only on the 8-10th day after the operation (only with independent urination). The stitches dissolve on their own, as a catgut thread is used.

If after removing the catheter, urination with a significant amount of blood in the urine occurs for two or three days, you should immediately contact your doctor.

Complications

The following complications may occur after this operation:

  • infection of the genitourinary system;
  • damage to organs or tissues as a result of surgery;
  • diverticulum;
  • hair getting into the urethra;
  • inflammation in the incision area;
  • recurrent stricture recurrence.

Indications

Surgery on urethra is done in the following cases:

  • urinary incontinence;
  • urethral strictures of various localizations;
  • tumors in the ureters and bladder;
  • urolithiasis;
  • cystitis;
  • urethral obliteration;
  • bladder cancer;
  • urethral calculus of any size.

Contraindications

There are the following contraindications to this operation:

  • blood clotting disorder;
  • patient weight exceeding 125 kg;
  • renal dysfunction;
  • cardiovascular disease (in in this case, the patient may not be able to withstand the anesthesia).
