General information

The causes of such a disease are often: disorders of the pyelourethral segment, stones in the ureter, excessive mobility of the kidney (nephroptosis). Therefore, this surgical intervention is carried out in order to normalize the correct outflow of urine from the kidney. There are two types of laparoscopic ureteropelvic junction plastic surgery:

  • Resection plastic surgery (according to the Anderson-Hines method), in which the abnormal part of the ureteropelvic junction is excised and a connection is made between the pelvis and the ureter. In case of significant expansion of the renal pelvis, the excess part of its tissue base is also removed in order to again prevent obstruction of the urine outflow.
  • The Fenger operation is performed by dissecting the ureteropelvic segment lengthwise and suturing it horizontally, which in turn contributes to the expansion of the segment connecting the ureter and the pelvis.

Preparation

First, a CT scan is performed to determine the general condition of the patient's body. MRI is also possible. It is mandatory to perform renal scintigraphy in order to determine the functioning of the damaged kidney and the stage of its pathological deviation.

Laparoscopic ureteropelvic junction plastic surgery is one of the most effective operations for treating disorders of this part of the human urinary system.

If earlier laparoscopic plastic surgery was performed using one incision in the side, today this operation is performed using 3-4 punctures in the anterior abdominal wall in the following sequence:

  • insertion of instruments into the abdominal cavity, mobilization of the colon and access to the kidney;
  • isolation of the ureter and renal pelvis;
  • installation of a special tube (stent) in the ureter to ensure the patency of the segment (for several weeks);
  • installation of a drainage tube for one or two days to ensure that there is no urine leakage from the reconstructed segment.
Special tubes (stent and drainage) are removed in the hospital. From two to seven days, the patient also walks with a catheter to prevent urine from getting into the recovery area.

Rehabilitation period

After the operation, a comparison is made (after six weeks), and renal scintigraphy is also performed again, which is necessarily compared with the study before the operation. This is done in order to identify improvements in the outflow of urine. In more than 90% of cases, laparoscopic ureteropelvic junction plastic surgery provides excellent results and significant improvements.

Indications

This type of surgery is indicated in the event of a functional obstruction in the ureteropelvic junction, which in turn leads to a slowdown in the natural flow of urine through the urinary tract and expansion of the renal pelvis and calyces.

Contraindications

Complications
