Pyelolithotomy is a surgical procedure to remove stones in the ureter and kidneys using an open procedure.

General information

An analogue of pyelolithotomy is ureterolithoextraction, which is performed endoscopically using 3-4 holes made in the skin.

Pyelolithotomy is performed only in in cases where surgical intervention cannot be avoided. In cases where kidney or ureter stones can pass through another route, a urologist will prescribe the appropriate treatment.

This operation is performed by experienced specialists under general anesthesia for 1-3 hours.

The process of the operation

Preparation

Preparation is mandatory, since the operation is performed under general anesthesia. Before the operation, the patient must pass general clinical tests (general urine and blood tests, electrocardiography, biochemical testing). Intravenous urography is also mandatory. This procedure allows the attending physician to determine the size and location of the calculus.

Pyelolithotomy procedure

The attending physician first administers anesthesia to the patient. The surgeon makes an incision that should run parallel to the costal arch. Its length should be 10 cm. Access to the kidney or ureter is opened by moving apart all tissue layers. An incision is made at the site of the stone projection. Then the stone is removed. Then the ureter or kidney is sutured at the site of the stone projection with self-absorbable materials. In case of suppuration at the site of the stone, a special catheter is inserted into the kidney, which is capable of removing urine during the treatment period. At the end of the procedure, the surgeon performs layer-by-layer suturing of the wound. Transferring the patient from the operating room to the intensive care unit.

Rehabilitation period

The rehabilitation period after pyelolithotomy is very important for the patient, so he must strictly follow the following recommendations:

  • After pyelolithotomy, the stitches can be removed on the 10th day, so during this time the patient should limit physical activity and strictly follow the doctor's instructions.
  • In the postoperative period, the patient may need painkillers, which are prescribed by his attending urologist. Their choice will depend on the intensity of the patient's pain.
To quickly return to normal after pyelolithotomy, experienced urologists advise starting to walk along the department corridor on the second day after the operation.
  • There are no strict instructions regarding nutrition. The only thing that can be increased is water consumption. This is necessary so that the body can naturally flush the urinary system from mucus plugs, microorganisms and blood clots.
  • Within one month after the operation, it is necessary to use herbal preparations prescribed by the urologist. They are able to restore the patient's immunity after surgery, and also have an antimicrobial effect.
  • The patient can return to his previous rhythm of life no earlier than two months after pyelolithotomy.

Indications

Pyelolithotomy is performed in the following patient conditions:

  • the presence of large stones in the ureter, so large that they cannot pass through the opening on their own;
  • the presence of large stones in the renal pelvis;

Contraindications

Pyelolithotomy has its own contraindications. It is not performed in the following conditions:

  • anemic conditions of the patient;
  • impaired blood clotting function;
  • serious disorders in the cardiovascular system;
  • regular use of blood thinning medications;
  • the presence of inflammation in the body or an infectious disease.

Complications

All possible consequences of pyelolithotomy can be characterized as follows:

  • General surgical - those complications that can occur as a result of any surgical intervention and are entirely dependent on the patient's body. This may be cardiac arrest, an allergic reaction to anesthesia, bleeding during surgery, or an exacerbation of a chronic disease.
  • Complications typical of pyelolithotomy: ureteral injury, adhesions of the perirenal space, inflammatory processes in the urinary tract, accidental cutting of the vessels that feed the urinary organs.
