General information

Depending on the method of the operation, nephrectomy is divided into two main types:

  • removal of the kidney using traditional open nephrectomy;
  • removal of the kidney laparoscopically.

The main difference between laparoscopic kidney removal surgery and traditional nephrectomy is that laparoscopy is performed through small holes no larger than 2 cm in size, which the doctor makes in the front and side of the abdominal wall, that is, without making any incisions. After the punctures, the surgical instrument is inserted into the part of the body that is undergoing surgery. It is the use of this method that makes the patient much easier and more comfortable to endure the postoperative period.

Kidney removal is performed in case of any pathologies in its functioning or in case of impossibility of restoring its primary integrity, which often gives the risk of serious complications.

Procedure

Preparation

Immediately before the surgical intervention, each patient must take blood and urine tests. Before the operation, the patient must be examined by an anesthesiologist, because this surgical procedure is one of the most serious and is performed under general anesthesia.

The process of performing a nephrectomy

During the operation, the patient is usually placed on the healthy side, fixing this position with the help of devices on the operating table and special rollers. An incision is made in layers on the patient's body in the lumbar region, which allows the doctor to get to the kidney. The doctor pulls the kidney out of the so-called fat capsule and takes it out through the incised wound. Then the kidney stalk is bandaged and cut, as well as its ureter, artery and vein, after which the kidney itself is completely eliminated. After the bleeding of small vessels has stopped, the doctor installs a drain in the place where the kidney was. The wound is also sutured in layers and a sterile bandage is applied on top.

Rehabilitation period

In the postoperative period, the patient is treated with antibiotics and painkillers. The drain is removed after a few days after the operation. The bandage should be changed regularly. The sutures themselves are usually removed on the 10th day after surgery.

Indications

Indications for nephrectomy are:

  • kidney damage caused by a gunshot wound with destruction of the tissue structure of the organ;
  • closed injuries with a serious rupture of the kidney;
  • purulent-destructive damage to the kidney as a consequence of the course of urolithiasis;
  • malignant tumors of the kidney.

Contraindications

Surgery to remove the kidney is strictly contraindicated:

  • persons with a severe general condition, unless it is caused by kidney pathology;
  • if there is only one kidney;
  • if the second kidney is affected, which will also soon cease to function normally.

Complications

During this surgical procedure, certain complications are possible:

  • heavy bleeding;
  • damage to the abdominal wall or pancreas (due to the immediate proximity to the kidney).

Among the postoperative complications that occur extremely rarely, but still happen, it is necessary to name: heart attacks, strokes, pneumonia, thrombophlebitis (inflammation of the vein wall and the formation of a blood clot), atelectasis (collapse of the lobe of the lung), pancreatitis (if the pancreas was affected), paralytic intestinal obstruction.
