Therapeutic block is a relatively "young" treatment method, characterized by injections into the nerve plexus. The goal is pain relief; relief of spasm, inflammation, swelling; diagnostics.
General information
The analgesic effect is achieved by temporarily disabling one link in the arc of the pain reflex. The procedure has a quick but temporary positive result. Sometimes, even with the help of magnetic resonance imaging, it is difficult to establish the correct diagnosis, since the source of pain is unclear. Then this manipulation comes to the rescue. The anesthetic drug acts on certain anatomical structures. If the pain sensations decrease or disappear after this, then it is in this structure that the pathology develops. Thus, diagnoses with similar symptoms are excluded.
Therapeutic blockade in neurology is the most effective. Its goal is to remove the cause of pain. It is divided into types depending on the injection site:
- cervical;
- brachial;
- thoracic;
- lumbosacral;
- intercostal;
- conduction;
- tibial nerve;
- sciatic nerve;
- epidural.
Advantages of this method:
- rapid achievement of analgesic, anti-inflammatory effect;
- relative painlessness of manipulation;
- minimal probability of side effects;
- possibility of repeated use;
- speed implementation.
Procedure
Preparation
This manipulation is performed by a vertebroneurologist, neurologist, surgeon, orthopedist. Of course, an accurate diagnosis is required first. The patient is examined and diagnostics are prescribed: X-ray, computed tomography, magnetic resonance, laboratory tests. Sessions are carried out both on an outpatient basis and in a hospital.
Procedure
- The patient is placed on a comfortable couch on his stomach. In some cases, a sitting position facing the back of a chair is necessary.
- The site of action is determined by palpation - the area of the greatest pain. It is treated with alcohol. Novocaine anesthesia is administered intradermally with a thin needle.
- A 0.5% novocaine injection is administered with a larger needle, penetrating deeper.
- Then, at a certain angle, the needle is advanced two cm deep and another 20 ml of novocaine is injected directly into the pathological focus. On average, the needle goes up to six cm deep.
The medicine is injected into several points in this way. After five to seven days, the procedure can be repeated if the first one gave a positive effect. A total of two to four repeated sessions are allowed.
A longer and more stable positive result is achieved by adding corticosteroid hormones to the anesthetic.
After the procedure, the patient needs to lie down for some time.
Indications
Therapeutic drug blockades are indicated for the following conditions:
- neuritis;
- radiculitis;
- intercostal neuralgia;
- myositis;
- sympathalgia;
- osteochondrosis;
- herniated intervertebral discs;
- other neurological disorders and pathologies of the spine.
Contraindications
- elevated body temperature;
- hemorrhages;
- infection at the site of manipulation;
- severe cardiovascular failure;
- renal, hepatic failure;
- allergy to drugs;
- peptic ulcer in the acute stage;
- severe pathologies of the central nervous system.
Complications
The consequences of therapeutic blockades occur in less than 0.5% of cases. These may be:
- intoxication caused by drug overdose;
- immediate or delayed allergy;
- vegetative-vascular disorders;
- infection due to failure to observe asepsis;
- local inflammatory reactions;
- nerve, vessel, tissue injuries.
