General information

Such interosseous structures in the body are present in the temporomandibular, acromioclavicular, sternoclavicular and knee joints. These cartilaginous formations feel the greatest load in the knee, therefore pathological changes are more common in this section.

The cause of ruptures and deformations are microtraumas, metabolic disorders and significant loads.

Therapy process

In case of knee injury, the patient is given painkillers, a splint is applied and sent to the trauma department.

Preparation

Five to seven days before the appointed date, the patient should take care to exclude alcohol and heavy foods from his diet.

Before the manipulations, the patient's blood and urine are taken, tests are carried out for bilirubin and sugar, the absence of infectious diseases is checked. In addition, the patient's electrocardiogram is taken, the blood type and Rh factor are determined.

Surgery on the meniscus of the knee joint is indicated for its ruptures and crushing, as well as when conservative treatment has not yielded noticeable results.

Surgery can be performed using an open method or a minimally invasive method with minimal incisions (arthroscopy).

Open surgeries

They are performed extremely rarely, mainly to cut out the meniscus in very severe cases. The patient is given local infiltration anesthesia. The knee section is cut layer by layer. Before removal, an additional anesthetic is injected into the cavity. The cartilaginous insert is grasped using a Kocher clamp and cut off with a scalpel in the area of the posterior horn, where it is attached to the joint capsule. After this, the wound is sutured layer by layer.

Minimally invasive intervention is usually preferred to open surgery.

Arthroscopy

This manipulation involves making two punctures in the knee area: one for inserting the arthroscope, the second for endoscopic instruments. Arthroscopy is usually performed under epidural anesthesia, during which the patient does not feel pain below the lumbar region.

Depending on the severity of the deformities, one of the treatment methods is chosen: restoration, removal or transplantation of the shock-absorbing element.

During the restorative measures, the cartilaginous structure is sutured with a special suture. The choice of suture is left to the surgeon and depends on the specific case. Fixation with absorbable fixators is widely used. First, all deformed and dead structures are removed, and the edges of the cartilage are processed.

During restorative procedures, they try to preserve the body of the natural pad as much as possible to prevent the development of arthrosis in the future.

Removal of the shock-absorbing layer is performed in severe cases of crushing, tearing off a large part of the tissue and developing complications. In modern medicine, they try to remove only the damaged part, aligning the edges of the remaining fragment.

Achievements of modern medicine make it possible to replace a damaged shock-absorbing disc with a donor transplant or synthetic implant. Donor replacement material is inserted through a small incision. It is installed in place of the damaged one and sewn on. Artificial cartilage is made of a spongy material through which blood vessels easily penetrate, which promotes the formation of natural tissues in place of damaged ones.

Indications

Indications for surgical interventions are data from X-ray examination with contrast, magnetic resonance imaging and examination using an arthroscope.

Complications

In almost 90% of cases, manipulations are successful. Rarely encountered are the following types of complications: internal bleeding, thrombosis, infectious inflammation, nerve damage.

Contraindications

Invasive procedures are not performed in the presence of the following factors:

  • acute inflammatory processes;
  • degenerative changes;
  • general diseases.
