General information

The intervertebral cartilaginous structure provides support and movement for the body's skeleton. It consists of a gelatinous nucleus, connective tissue plates that have a ring-shaped form, and end plates.

The cartilaginous connection between the vertebrae acts as a shock absorber. The ring-shaped plates prevent horizontal displacement of the spine elements and take an active part in metabolism. The nucleus pulposus is responsible for load distribution and plays an important role in the transport of nutrients.

The discs and facet joints formed by the articular processes ensure the mobility of the spine segment, in addition, they take on 20-30% of the load when a person is in a standing position.

Osteochondrosis, the treatment of which should be carried out using complex methods, can be a consequence of pathological changes in various body systems: endocrine, nervous, humoral. The disease can also be caused by external factors, such as excessive overloads, injuries of various origins, etc. They lead to disturbances in metabolic and microcirculatory processes inside the tissues.

Scientists associate the development of pathological changes in the intervertebral cartilage with low water absorption and imbalance of biochemical processes. With timely detection of developing pathologies, treatment of osteochondrosis of the spine by reducing the impact on the affected segment and restoring trophic systems would give the desired effect.

Depending on the degree of deformation, the following stages of disease development are distinguished:

  • First stage: internal displacements are observed, cracks in the connective tissue ring appear.
  • Second stage: protrusion (bending) of the disc, almost complete rupture of the connective tissue ring.
  • Third stage: loss of parts of the gelatinous nucleus, hernia.

Procedure

Treatment of osteochondrosis of the spine depends on the level of development of the disease.

Thus, at the first stage of severity, when only displacements inside the disc are observed, drug therapy is prescribed: non-steroidal anti-inflammatory drugs and muscle relaxants. In case of severe pain, steroid anti-inflammatory drugs are also prescribed.

The patient is recommended massage and stretching of the supporting part of the body on a special table or in water, as well as fixation of problem areas with a corset.

The second stage of the disease can also be treated with medication or physiotherapy methods.

In case of severe pain syndrome and tendencies for disease progression, surgical intervention is indicated.

The surgeon can use both minimally invasive techniques and open surgical procedures.

Minimally invasive intervention is performed through a small incision. Deformed parts are removed with special instruments.

Open surgery is performed when the damaged intervertebral space needs to be replaced with a transplant. The latter can be artificial materials and autotransplants obtained from the patient's own tissues.

Decompression and stabilization operations are performed in case of degenerative processes, arthrogenic instability and narrowing of the spinal nerve canals.

Indications

Indications for therapy are the results of clinical and instrumental examination confirming the presence of deviations from the norm. X-ray examination is used as the main diagnostic method. From the images, the doctor can see a complete picture of structural changes in the intervertebral spaces. If it is impossible to perform an X-ray examination, tomography is used.

Complications

Sometimes surgical interventions are fraught with unpleasant consequences, in particular, the following may be observed:

  • recurrence of a herniated disc of intervertebral cartilage;
  • postoperative scars and adhesions;
  • limited mobility in joints when replacing cartilaginous structures with a transplant;
  • disruption of muscles, ligaments, etc.
