General information
Generally accepted classification of fungal pathogenesis:
- Dermatomycosis (dermatophytosis, dermatophytosis, trichophytosis, onychomycosis) is a skin inflammation caused by contagious parasitic fungi Trichophyton, Microsporum and Epidermophyton.
- Candidiasis is a variety of pathologies of the epidermis and epithelium, nail plates and internal organs caused by yeast-like microscopic fungi of the genus Candida present in normal healthy microflora.
- Deep mycosis is a non-contagious infectious lesion of internal organs (aspergillosis, blastomycosis, histaplasmosis, geotrichosis, coccidioidomycosis, cryptococcosis, mucormycosis, rhinosporiosis, chromomycosis).
- Keratomycosis (lichen, piedra, scab, sporotrichosis) is a lesion of the hair and stratum corneum caused by yeast-like fungi Pityrosporum, Exophiala, Piedraia, Trichosporon, Sporotrichium.
Fungal infections are highly polymorphic, so the search for causes and determination of sources is carried out using a set of differential diagnostic tools:
- microscopic;
- cultural;
- luminescent;
- histological;
- immunological.
Diagnostics of fungal diseases
- Microscopic - performed by means of microscopy, for which the material being studied is developed with dyes with their maceration. It reveals budding cells and filamentous hyphae and allows to judge only the nature of infection, without giving a full idea of the contagion itself. The efficiency has an error and directly depends on the level and qualifications of the personnel.
- Cultural – effective for achieving the goals of recognizing latent forms and dermatophytes carried by healthy people. It reveals and recognizes the type of contagion even with negative microscopy results. Purity varies within 30-50%, subject to compliance with the required standards. Pathogen seeding is performed in Sabouraud's medium in the presence of antibiotics that suppress the growth of contaminants entering with air, with subculture in selective media. The updating process is completed by microscopic analysis.
- Luminescent diagnostics of fungal diseases with a Wood's lamp. It is based on a physical effect - fluorescence of a substance infected with Microsporum and Trichophyton spores in the "soft" long-wave ultraviolet spectrum (365, 398, 404 and 407 nanometers), which manifests itself when the radiation flow passes through a light filter of blue-violet "black" uviol glass. The objects under study, infected with Microsporum, exhibit bright luminescence, while those exposed to Trichophyton react with a dim greenish glow. The nature of this phenomenon has not been fully studied, but it is known for sure that after such exposure, the infected substance "leaves a trace" - the objects continue to emit fluorescent glow for a fairly long period of time even after the death of the pathogens. Subsequently, the identified cultures are subjected to additional cultural research.
- Immunological - this is an authentic identification of allergic reactions based on the results of Pirquet, Moro and Mantoux tests, with the detection of antibodies by the interaction of serum and antigens. The final comparison of diagnostics provides an up-to-date and reliable perception of the process with the predicted dynamics of development.
- Histological - with preparations that change color, it is able to detect polysaccharides of chitin and cellulose in the cell walls of most known dermatophyte fungi.
Indications
Manifestations of fungal infections begin with hyperemia, dryness and desquamation of scales with the formation of microcracks and a specific floury coating. The fungus affects:
- the skin of the soles and interdigital folds, which becomes dry and red, with abundant peeling;
- the nail plates, subjecting them to destruction and hyperkeratosis;
- smooth skin and areas of natural folds - armpits, mammary glands and groin area.
Contraindications and complications
There are no side effects when performing the procedures.
Long-term therapy with antibiotics or hormonal drugs contributes to the provocation of side effects and negates the research.
