General information
The P1NP marker will allow you to judge the speed and direction of bone metabolism processes. The level of P1NP in the blood is higher at night.
The organic matrix (base) of the bone is represented mainly by type 1 collagen, which is formed from type 1 procollagen synthesized by fibroblasts and osteoblasts. The N-terminal propeptide of procollagen 1 is released into the intercellular space and bloodstream during the formation of collagen 1 and its incorporation into the bone matrix.
Procedure
Preparation
You must not eat for eight hours before the procedure. Emotional stress and physical activity should be completely excluded the day before the session. You must not smoke half an hour before the test.
Bone matrix formation marker procedure
The procedure involves taking blood from a vein. The serum remains stable for 7 days at 2–8 °C. Avoid thawing and refreezing twice. It is recommended to conduct the study before the start of therapy and then at intervals of three months from its start.
Indications
The event is indicated:
- Before the start of osteoporosis therapy.
- When examining patients with Paget's disease, osteoporosis.
- To assess the effectiveness of anabolic and antiresorptive therapy.

- over 19 years – from 8 up to 80 ng/ml.
For men:

- from 1 to 10 years – from 184 to 1024 ng/ml;
- from 10 to 12 years – from 237 to 635 ng/ml;
- from 12 to 15 years – from 202 to 1212 ng/ml;

Elevated level:
- osteoporosis;
- osteomalacia;
- osteogenesis imperfecta;
- Paget's disease;
- renal osteodystrophy;
- metastatic bone lesions.
The P1NP level should be assessed taking into account the indicators of tissue resorption and remodeling in dynamics. An increase in P1NP during antiresorptive therapy indicates the effectiveness of treatment measures.
The results in patients with impaired renal function should be interpreted with caution, since there may be a decrease in the excretion of the marker in the urine and, consequently, an increase in its level in the serum.
