General information

Laparoscopic adrenalectomy can be performed using various techniques, depending on the size, area, and type of lesion. The posterior approach is suitable for small, unilateral adrenal tumors. The anterior approach is mainly used for bilateral adrenalectomy. Retroperitoneal and transperitoneal approaches are used. The advantage of this method is that it is minimally invasive and cost-effective.

Procedure

Laparoscopic adrenalectomy procedure

The patient's position depends on which side the surgery will be performed on. Most often, the patient lies on his back in the "on his side" position. The abdominal cavity is examined. For this, a special needle is inserted and a certain pressure is created. Then three more trocars are inserted. First, the Told line is determined, along which the peritoneum is dissected, and then the ligaments. The upper pole of the kidney is mobilized. The adrenal gland is identified by its golden color, then it is mobilized. Depending on the size of the adrenal gland, it is removed either through one of the ten-millimeter trocars, or on a sealed bag with a ten-millimeter widening of the wound with one of the trocars. The pressure in the peritoneum is reduced and the area of the operation is examined. If bleeding vessels are detected, hemostasis is performed.

Drainage is sometimes installed in the retroperitoneal space. Then the wounds are sutured. The duration of the entire procedure is about three hours.

Rehabilitation period

The patient is allowed to drink water six to eight hours after the operation. Patients are usually discharged on the second to fifth day. It becomes possible to lead a normal life after eight to ten days.

Indications

Absolute indications for laparoscopic adrenalectomy are:

  • angiomyolipomas;
  • small, hormonally inactive adrenal cancer;
  • growing, hormonally inactive adenomas;
  • adrenal cancer with the development of Conn's syndrome;
  • pheochromocytomas;
  • aldosterones;
  • medullary adrenal cysts.

Contraindications

Absolute contraindications include:

  • intestinal obstruction;
  • blood coagulation disorder;
  • peritonitis.

Complications

The main complication during surgery is the possibility of bleeding. In most cases, it can be stopped conservatively.
