General information

Most often, such an intervention is optimal for a unilateral thyroid defect. In other cases, resection, thyroidectomy are more appropriate. The normal size is measured by the phalanx of the thumb.

Right hemithyroidectomy is performed under general anesthesia with the exception of local anesthesia.

Procedure

Preparation

First, laboratory tests are prescribed:

  • General and biochemical blood tests, determination of its coagulability, hepatitis C and B, syphilis, HIV infection, hormone levels (TSH, T4 and T3, antibodies to TPO).
  • Ultrasound of the gland and lymph nodes in the neck.
  • Computer tomography, fine-needle aspiration biopsy.
  • Histology and scintigraphy.

An ENT examination for vocal fold laryngoscopy is also required. An electrocardiogram is performed if necessary.

Before the surgery itself, markings are made on the skin using a special marker. This will help to evenly stitch the edges after the incision is made in the operating room. An inconspicuous symmetrical scar will remain in the neck area.

Hemithyroidectomy procedure

The incision is made on both sides of the mid-cervical-midline, adhering to the horizontal direction. The size and length of the cut should not exceed several centimeters. The neck muscles are spread apart and access to the manipulation area is opened.

First, the vessels of the upper poles are cut and the treatment site is examined to exclude injury to the branches of the superior laryngeal nerve. It is located behind the gland tissue. Then it and other nerve endings necessary for human activity are isolated and part of the organ is removed.

The stitching is done layer by layer and drainage is left. A cosmetic suture is formed on the skin with the application of a non-absorbable (if it needs to be removed later) or absorbable (if this is not necessary) material.

Rehabilitation period

After hemithyroidectomy, a specialist monitors hormone levels to prevent hypothyroidism. They also pay attention to compensating for their deficiency with medications. Most often, such drugs must be taken throughout life.

Indications

The technique is typical for those who have:

  • nodular forehead;
  • thyroid adenoma;
  • follicular cancer.

Contraindications

Therapy is prohibited for people with:

  • the spread of the tumor process beyond one part of the thyroid gland;
  • chronic diseases in the terminal stages;
  • decompensated state of the respiratory and cardiovascular systems;
  • oncology in other areas.

Complications

The following consequences may occur:

  • damage to the recurrent laryngeal nerve due to paresis of the vocal folds, resulting in complete or partial loss of voice;
  • hypoparathyroidism (decreased function of the parathyroid glands, which leads to a decrease in the formation of parathyroid hormone);
  • loss of sensitivity of a section of the mucous membrane of the larynx;
  • development of a loose fit of the epiglottis to the entrance to the throat, which causes choking on food while eating;
  • deviations in the timbre of the voice (change in frequency, tone);
  • suppuration of the wound, bleeding from it.
