General information

Adrenalectomy is performed in case of tumor damage to the adrenal gland. Partial adrenalectomy is used for benign tumors – tumor removal while preserving healthy gland tissue. During the operation, a histological analysis of the removed element is performed and it is determined how adequate the volume of intervention was. In case of malignant growth, lymph nodes and surrounding tissue are additionally removed. Traditional unilateral adrenalectomy is usually performed through the lumbar approach.

Procedure

Preparation

The patient is prescribed the following tests:

  • ultrasound of the abdominal cavity;
  • mandatory medical examination;
  • all blood and urine tests;
  • radiation scanning;
  • computed tomography of the abdominal cavity;
  • MRI;
  • computed tomography of the head.

You should stop taking certain medications a week before the procedure.

You must not eat after midnight on the day of the procedure. Your blood pressure will be checked before the procedure. If necessary, the specialist will stabilize it to a normal level.

Adrenalectomy procedure

The adrenal glands, which have a large mass, are removed through the front of the abdominal cavity. This is done so that they can be easily removed.

An incision is made only in the abdominal cavity or under the rib cage. The adrenal gland is very carefully separated from the kidney, after which the gland is removed, and the incision is fastened with staples or stitched.

Rehabilitation period

For some time, you may feel nauseated. The doctor may insert a special nasogastric tube through the nose into the stomach to remove gastric juice. In this case, the patient receives all the necessary nutrients and medications intravenously.

The recovery period can take from four to six weeks. For complete recovery, the patient must monitor their weight daily and inform the attending physician if it increases by more than a kilogram. It is also necessary to follow a diet and control blood pressure.

Indications

Indications for the intervention may be:

  • adrenal gland diseases;
  • large adrenal gland mass;
  • production of very large amounts of hormones;
  • causeless adrenal gland disorder.

Contraindications

Absolute contraindications are:

  • terminal conditions of the patient, coma;
  • decompensation of cardiopulmonary activity in the process of progression;
  • sepsis or diffuse purulent peritonitis;
  • other pathologies and conditions with a high risk of surgical intervention.

General contraindications are:

  • late stage obesity;
  • poor blood clotting;
  • late pregnancy;
  • diffuse or local peritonitis, as well as suspicion of it;
  • infectious diseases.

Local contraindications may be:

  • infectious process in the anterior abdominal wall;
  • previously performed open operations on the abdominal cavity, adhesions in the abdominal cavity or cicatricial deformities of the abdominal walls.

Complications

Possible consequences:

  • low blood pressure;
  • insufficient cortisol production;
  • bleeding;
  • blood clots in the legs;
  • infectious processes in the lungs, kidneys or urinary tract;
  • injuries in organs near the intervention area;
  • various adverse reactions to the anesthetic drug.
