General information
The sphincter apparatus holds the contents of the intestine. If it is disrupted, gases, solid and liquid contents of the intestine will come out.
When the sphincter (closing) apparatus is normal, it freely holds all the contents of the intestine in various body positions, coughing, sneezing and physical activity.
There are several stages of this disease:
- At the first stage of clinical manifestation of sphincter apparatus insufficiency, gases that are in the intestine are no longer held in it.
- During the second stage of sphincter dysfunction, the patient cannot hold liquid feces.
- The third degree of disorder affects all the contents of the intestine. The damaged sphincter apparatus cannot hold either gases or liquid or solid feces.
Stages 2 and 3 of sphincter apparatus dysfunction require surgical intervention.
Procedure
Preparation
This operation requires complete bowel cleansing. For this, you can use tablets recommended by your doctor or use an enema. Depending on the condition of the intestine, the urologist can use different methods to clean it.
Sphincteroplasty procedure
Introduction of anesthesia to the patient's body. Most often, sphincteroplasty is performed under general anesthesia, but there are exceptions. This largely depends on the patient's condition and the complexity of his disease. Sphincteroplasty is performed in several stages:
- Two flaps on legs are cut out from the levator muscles.
- The legs are formed at 6 and 12 o'clock.
- At the site of the sphincter apparatus defect, a flap with a leg at 6 o'clock is first laid out. It is carefully laid out on the lateral wall of the rectum.
- The flap with a fragment of the remaining part of the anal sphincter muscles is sutured.
- The flap with a leg at 12 o'clock is placed on the muscle ring.
- The free edge of this ring is fixed to the sphincter muscles on the flap that was laid out earlier.
- In case of wounds, the sphincter apparatus is sutured using catgut threads. This allows you to restore the mucous membrane and stop bleeding.
- The use of muscle flaps allows you to replace the defect of the anal sphincter apparatus.
Rehabilitation period
Sphincteroplasty does not have serious complications, so after it is performed, the rehabilitation period, as a rule, passes relatively quickly. In order for the wounds to heal quickly and the sphincter to function normally, the patient must follow these recommendations:
- use of special healing ointments;
- use of a gas outlet tube;
- physical therapy.
If the operation is successful, on the 6-8th day, feces begin to come out using an enema, and on the 20th day, the patient can go to the toilet on his own.
The patient must be under constant supervision of a urologist.
Indications and contraindications
Sphincteroplasty is used in all cases of sphincter apparatus dysfunction.
In addition, other indications for sphincteroplasty may be cases:
- violation of the anatomical structure of the sphincter apparatus;
- sphincter defect, which can extend up to a quarter of the entire circumference of the anal canal.
Contraindications
Sphincteroplasty is contraindicated in case of damage to the peripheral and central nervous system, which affect the functionality of the pelvic organs.
Complications
Sphincteroplasty has a number of complications:
- The appearance of infection in the area of the sphincter apparatus;
- colic in the lower abdomen;
- blood in the anal area during stool.

Sphincteroplasty should be performed only in specialized clinics and such an operation should be performed by an experienced urologist surgeon. The cost of sphincteroplasty depends on the complexity of the sphincter apparatus and the price of the operation ranges from 10 to 20 thousand rubles.
