General information
Measles is included in the group of childhood infectious diseases. Susceptibility to the pathogen is high, it is transmitted by airborne droplets, the disease is severe. Characteristic are a high temperature reaction, abundant staged rash on the skin, leaving behind temporary pigmentation. The disease is accompanied by painful inflammatory changes in the nasopharynx and severe conjunctivitis, gives severe complications due to the addition of bacterial flora and a sharp decrease in immunity. Most often these are pneumonia, otitis, myocarditis, meningitis and others. Today it occurs in isolated cases.
Vaccination
At 12 months, it is administered subcutaneously into the outer surface of the upper third of the shoulder. Older children are allowed to inject into the subscapular region. The vaccine is live, weakened. The dry component is contained in a separate ampoule from the solvent. It is connected to it immediately before use. The kit may contain a syringe. A single-dose form is used. But there is also a multi-dose option for collective administration (from which the same 0.5 ml is taken per person).
Indications
The national calendar provides for groups and dates for vaccination:
- at 12 months with revaccination at 6 years for all healthy children;
- teenagers who are not vaccinated and have not been sick are immunized at 13 years;
- adults under 35 who have not had the disease or have no information about vaccinations;
- contacts with a sick person without vaccinations due to age or medical exemption are administered the vaccine no later than the 5th day of contact (1 dose). In some cases, it is permissible to administer measles immunoglobulin in the first three days of contact with the patient;
- in the event of a measles outbreak, even to all vaccinated people once.
Preparation for vaccination
A mandatory examination by a pediatrician (therapist), thermometry are carried out. Depending on the category of indications, various drugs are used. There are monovalent vaccines of Russian production, the French Ruvax. It is used mainly in the adult population. Children are currently prescribed two- or three-component vaccines - mainly the Russian Divax (measles + mumps).
If a child has already had one or even two infections indicated in a multi-component vaccine, its use is permitted after consultation with an immunologist.
A monovalent vaccine is combined simultaneously with other inactivated ones, with live ones - only after a month. After receiving gamma globulin or immunosuppressive therapy, it is done no earlier than three months later.
Currently, in controversial cases, it is possible to determine the protective antibody titer (the norm is 1:40) and make the right choice with the help of a qualified specialist.
Contraindications
Contraindications are divided into 2 groups: temporary and permanent.
Acute diseases, pregnancy are classified as the former.
Permanent ones include a severe allergic reaction to chicken or quail egg protein, aminoglycoside antibiotics, since they are used in the production of the vaccine.
Complications
It is well tolerated, but reactions are possible in the form of an increase in temperature on the 4th-7th day, slight local pain, and compaction in the injection area. Rarely, the clinical picture of mitigated measles develops (after 7 days): low temperature, quickly passing rash, mild signs of intoxication. Such a child does not pose a threat to others, the symptoms disappear in 2-3 days practically without treatment. Consultation with a pediatrician is required.
Rehabilitation
Not required. It is advisable not to contact other infections for a month for a favorable course of the vaccination period.
