General information

This intervention is a very traumatic procedure. To perform it, the doctor must take into account the general health of the patient, the nature of the pathological neoplasm and the functioning of the organs that will be operated on.

Procedure

Preparation

Before the surgery, the patient performs the following list of actions:

  • takes a general blood test;
  • takes a biochemical blood test;
  • takes a test for the cancer antigen CA 19-9;
  • undergoes an ultrasound examination or computed tomography, as well as an endoscopic examination of the bile ducts;
  • stops taking blood thinners;
  • refrains from eating and drinking for 6 hours before surgery.
After resection, the removed parts are necessarily sent for histological examination in order to develop a further treatment plan.

Pancreas resection procedure

Partial resection is of two types depending on the area where the tumor is located:

1. According to the Whipple method (when the head is affected):

  • the patient is given general anesthesia;
  • after he falls asleep, the doctor examines the area where the operation is planned using a laparoscope;
  • then an incision is made to gain access to the surgical field, after which the doctor blocks and removes the vessels that feed the organ being operated on, and removes the head affected by cancer;
  • sometimes it is also necessary to remove the gallbladder, certain bile ducts, a small area of the duodenum, stomach and nearby lymph nodes;
  • ultimately, the doctor connects the body of the gland, stomach, bile ducts and the center of the small intestine.

2. Distal surgery (formation in the tail area). This technique involves the following:

  • the patient is also given general anesthesia;
  • an incision is made, the blood vessels of this part are blocked and its tail is cut off;
  • it is also possible to cut out the spleen, its vessels and the head itself.

Rehabilitation period

The patient stays in intensive care for 24 hours after the operation. After the patient is transferred to the ward, he is recommended to get up and walk a little. Recovery and hospitalization usually require up to 2 weeks. The patient is advised to limit any physical activity for 3 months, as well as to carry out special hygienic treatment of the surgical wound. It is especially important to follow the rules of nutrition: frequent meals in small portions and fluid intake of about 2 liters.

Patients who have undergone partial resection require intravenous insulin due to changes in the functioning of the pancreas.

Indications and contraindications

Resection is performed in the following cases:

  • injury to the pancreas;
  • the presence of fistulas, tumors and cysts;
  • malignant neoplasm and the need for lymphadenectomy;
  • relapse of chronic pancreatitis.

Contraindications

This procedure is contraindicated in the following clinical situations:

  • metastasis of the tumor in the abdominal cavity;
  • tumor damage to the mesenteric root, visceral arteries.

Complications

The operation is fraught with the following complications:

  • bleeding;
  • infectious processes;
  • damage to nerves and nearby tissues;
  • the occurrence of diabetes mellitus;
  • pancreatic insufficiency.
