General information
This operation is a very radical method in the treatment of oncological disease of the head of the pancreas, which also affects the preampullary part of the common bile duct and the large papilla duodenum. During the intervention, the head of the organ and the duodenum are excised and the patency of the gastrointestinal and biliary tracts is restored.
Procedure
Preparation
Before the operation, the patient undergoes a list of mandatory tests: blood tests (general and biochemical), analysis for cancer antigens CA 19-9, ultrasound or CT, endoscopic examination of the biliary tract. Also, one week before the surgery, the patient is advised to stop taking blood thinners.
Pancreatoduodenal resection procedure
There are many ways to perform a Whipple resection. The classical method involves the procedure in 2 main stages:

The sequence of the intervention is as follows:
- administration of general anesthesia to the patient;

- blocking and removing the vessels that feed the pancreas;
- cutting off the head affected by the cancerous tumor;
- removal of the gallbladder and some bile ducts;
- excision of part of the stomach and duodenum intestines;
- reconnection of the pancreas with the stomach, bile ducts and small intestine;
- layer-by-layer suturing of the abdominal wall.
Whipple's operation can be performed either laparotomicly or laparoscopically.
Rehabilitation period
After resection, the patient must be under the supervision of specialists in the intensive care unit, after which he is transferred to the surgery department. The patient stays in the hospital for about 2 weeks. After surgery, you should adhere to a special diet, which will be recommended by the attending physician. It is also recommended to take drugs that contain enzymes.
As a rule, the body is restored in 2 months. To prevent relapse, the patient may be prescribed a course of chemo- or radiotherapy.
Indications and contraindications
This operation is performed if the patient has:
- pancreatic cancer;
- periampullary cancer (oncological disease of the duodenum);
- chronic pancreatitis;
- various lesions of the head of the pancreas.
Contraindications
Pancreatoduodenal resection is contraindicated in the following cases:
- metastases of a cancerous tumor in the liver and peritoneum;
- metastases in the lymph nodes outside the zone operations;
- involvement of a venous vessel, celiac trunk, superior mesenteric artery and common hepatic artery in the tumor process;
- severe concomitant diseases.
Complications
Among the possible complications should be named:
- development of diabetes mellitus;
- formation of a through hole in the pancreas (fistula);
- paralysis of the stomach;
- disorders of the digestive system.
