Procedure

Preparation

Preparation for surgery includes the following:

  • general blood test taking into account platelets;
  • blood test to determine the prothrombin index;
  • determination of blood glucose levels;
  • establishment of blood type and Rh factor;
  • blood test for cancer antigens and HIV infection;
  • consultation with a therapist;
  • ultrasound examination.

Procedure endoscopic papillosphincterotomy

Before the procedure, the patient is placed on his left side, after which the following is done:

  • anesthesia of the oral cavity and pharynx with a lidocaine solution;
  • insertion of a special device through the mouth, esophagus, as well as the stomach and duodenum in order to examine the large duodenal papilla (MDP);
  • monitoring the entire process on the monitor, the doctor inserts a catheter into the common bile duct (bile duct), a certain solution is injected into the patient through the catheter in order to assess the condition and size of the ducts, as well as the presence and number of stones in them;
  • insertion of a papillosphincterotomy with a special metal string through which a current is started, with the help of which the tissues of the MDP are dissected;
  • stones are removed using called Dormia basket or later go away on their own.
Endoscopic papillosphincterotomy is performed in the surgery department in an X-ray room equipped for this purpose.

Rehabilitation period

If there are no complications, the patient stays in the hospital for about a day. All recommendations of the attending physician should be followed.

Indications and contraindications

This procedure is indicated for patients with the following pathologies:

  • choledocholithiasis (stones in the bile ducts);
  • papillostenosis (disease of the major duodenal papilla);
  • tumor in the bile ducts and in the CBD.

Contraindications

This operation is contraindicated in the following cases:

  • extended papillostenosis of the CBD, as well as the terminal section of the common bile duct, both benign and malignant;
  • significant expansion of the bile ducts;
  • non-biliary pancreatitis.

Complications

Among the possible complications after surgery are the following:

  • bleeding;
  • occurrence of cholangitis (inflammation of the bile ducts), pancreatitis (inflammation of the pancreas);
  • perforation (violation of the integrity) of the duodenum;
  • septic shock.
