General information
The disease in question belongs to the group of autosomal recessive inherited disorders. This type of enzymopathy occurs when the baby inherits one copy of the defective gene from each of the parents. As a result, there is a deficiency of enzymes that synthesize hormones of the adrenal cortex.
In medicine, there are three classic forms of the disease:
- Viril - characterized by excessive secretion of steroid male sex hormones and a deficiency of 21-hydroxylase.
- Salt-losing - its appearance is due to an insufficient amount of aldosterone, which ensures the maintenance of water-salt exchange between the external and internal environment of the body.
- Hypertensive - occurs quite rarely, develops due to a deficiency of 11-hydroxylase and is accompanied by strong androgenization phenomena.
In girls, such pathological deviations occur approximately 5 times more often than in boys.
The process of carrying out procedures
Preparation
Diagnostics is performed by an endocrinologist. The specialist conducts an initial examination and prescribes a number of necessary studies. Scintigraphy, radiography of the wrist joints and computed tomography are also used.
Treatment of adrenogenital syndrome
Depending on the time of diagnosis, two types of therapy are possible: drug and surgical. In classical forms, hydrocortisone or prednisolone tablets are used. The dosage and schedule of administration are determined by the attending physician individually depending on the age of the child and the degree of virilization. On average, the daily dose is no more than 10 mg, which does not lead to side effects from this drug.
Treatment of the salt-wasting type is based on parenteral administration of hydrocortisone. When the condition normalizes, fludrocortisone (50-200 mcg/day) is additionally prescribed.
Girls aged 4-6 years require surgical correction of the external genitalia.
The results of surgical or drug intervention are assessed by the following criteria:
- normalization of the child's development rate;
- stabilization of blood pressure;
- stabilization of the amount of 17-OH-progesterone in fibrinogen-free blood plasma.
Parents should remember that the prognosis is favorable with early detection and adequate therapy. Therefore, if characteristic symptoms occur, do not delay visiting a doctor.
Rehabilitation period
The rehabilitation period includes taking hormones and periodic observations by the attending physician, who, if necessary, will prescribe a repeat course of therapeutic measures.
Indications
Treatment of adrenogenital syndrome is prescribed by a doctor in accordance with diagnostic data (increased levels of ACTH and testosterone in the blood, 17-KS in the urine) and the patient's symptoms.
The general clinical picture of this disease is as follows:
- The appearance of male secondary sexual characteristics in girls (low voice, developed body muscles).
- Accelerated growth with a tendency to sharp stop.
- Delays in emotional and mental development.
In addition, the salt-losing form is characterized by dyspeptic disorders, dehydration, tachycardia and seizures.
Contraindications
There are no specific contraindications for the treatment of adrenogenital syndrome.
Complications
Sometimes long-term use of corticoids leads to the development of various complications: arterial hypertension, glucosuria, obesity, osteoporosis, water-salt metabolism disorders. Therefore, prednisolone is increasingly used at present, which allows avoiding such negative phenomena.
