What is the Zimnitsky Urine Test

Osmoregulatory function is the kidneys’ ability to concentrate fluid and maintain its balance at a certain level, regardless of the amount of water consumed daily. With low water intake, the kidneys concentrate urine; otherwise, they reduce its concentration and actively excrete it.

The test examines two main indicators:

  • Total volume of urine excreted daily;
  • Its density.

In some cases, the distribution of nighttime and daytime fluid volumes is also analyzed.

Based on the results, several diseases of the urinary, endocrine, and cardiovascular systems can be diagnosed:

  • Hyposthenuria or low density may indicate chronic kidney failure resulting from treated inflammatory diseases, diabetes insipidus, heart pathologies, or bilateral pyelonephritis.
  • Hypersthenuria or high density – a sign of glomerulonephritis in various forms or gestational toxemia in pregnant women.
  • Polyuria or exceeding the normal daily volume – diabetes mellitus, diabetes insipidus, or renal dysfunction.
  • Equalization of nighttime and daytime urine or predominance of the former over the latter indicates heart conditions.

Indications for Zimnitsky Urine Testing

The test is ordered by a general practitioner, pediatrician, nephrologist, urologist, gynecologist, endocrinologist, or family doctor.

It is indicated by suspicion of diseases of the endocrine, urinary, or cardiovascular systems:

  • Chronic or exacerbated pyelonephritis;
  • Glomerulonephritis;
  • Diabetes mellitus or diabetes insipidus;
  • Renal dysfunction;
  • Hypertension.

How the Zimnitsky Urine Test Is Performed

Urine is collected throughout the day (24 hours) at equal intervals. Six or eight containers are needed, sometimes twelve. These can be thoroughly washed and dried glass jars or pharmacy containers. Each container is labeled with the urination time.

During the day, keep a record of the amount of water, milk, tea, and liquid foods consumed. The first urination at 6:00 a.m. is not tested. Subsequent urinations, starting three hours later, are collected along with all others until 6:00 a.m. the next day.

Samples must be stored in a refrigerator and submitted to the laboratory in the morning.

During collection, avoid diuretics and maintain your usual fluid intake routine.
