General information
In During a bronchoscopy, one of two types of instruments is used for a child: a bronchofibroscope or a rigid respiratory bronchoscope. Depending on this, a distinction is made between rigid and flexible examination methods:
- Rigid - used to detect foreign objects (coins, large buttons, small toys), as well as in cases of obvious bleeding from the respiratory organs, for example, with tuberculosis. General anesthesia is used to carry out the process.
- Flexible - performed using a bendable bronchoscope. It is used more often, since anesthesia is not required in this case. It is used to examine the upper respiratory system from the inside (mucous surfaces of the bronchi and trachea, vocal cords, larynx, collecting tissue particles).
In addition to therapeutic, there is a method of diagnostic bronchoscopy, which is performed to clarify certain diseases or confirm a diagnosis. A catheter with a diameter of no more than 3 mm is used to examine children.
Procedure
Preparation
It is recommended to take blood and urine tests the day before the examination. The manipulation is carried out strictly on an empty stomach, usually in the first half of the day. No other special preparations are required.
Conduct
When using the flexible method, local anesthesia is used. The child sits or lies down. An anesthetic substance is injected into each nostril or oral cavity, which it is advisable to inhale deeply. After slight numbness, the specialist inserts the bronchoscope through the mouth or nose and advances it deep into the respiratory tract. In this case, there may be urges to vomit or a suffocating cough, which should be endured. The doctor must have a vomit aspirator on hand.
Not all small children can withstand such a manipulation. Therefore, the procedure usually takes place under short-term general anesthesia. As soon as the baby falls asleep, an anesthetic is injected into the vocal cords, and then into the bifurcation of the trachea. Considering that denaturation may occur during the manipulation with spontaneous breathing, the anesthesiologist also uses a laryngeal mask. Its use allows you to significantly expand the scope of the examination without affecting the child's condition. If the mask is used for children under 3 years old, then anesthesia is maintained using inhalation anesthetics.
Rehabilitation period
After the end, about half an hour, slight numbness of the surfaces and a hoarse voice remain. It is not recommended to eat or drink for three hours.
Indications
A child needs a bronchoscopy:
- if there is a suspicion of a malignant or benign tumor;
- in inflammatory conditions;
- to diagnose the causes of hemoptysis or pulmonary hemorrhage;
- if radiographic signs of disseminated processes are detected (spots or darkening of unknown origin);
- in tuberculosis;
- in purulent formations (gangrene, abscess);
- if a foreign body gets in for the purpose of removing it;
- in bronchial asthma;
- in case of narrowing of the bronchi during illness for their expansion;
- for taking tissue samples;
- for assessing the condition of organs after surgical interventions;
- for administering drugs;
- for collecting mucus or sputum for further study.
Main contraindications
The study should not be carried out in cases of mental illness, schizophrenia, epilepsy, acute stage of bronchial asthma, high blood pressure, allergic reactions to painkillers (lidocaine, novocaine) or anesthesia.
Complications
Due to the sufficient specificity of the procedure, the following consequences are possible:
- damage to the mucous walls by the device - occurs rarely, but can happen due to the incompetence of the health worker or restlessness patient behavior. Serious injuries require surgical intervention, minor ones - are restored on their own;
- opening of bleeding during biopsy - severe ones are eliminated surgically, weak ones (from small vessels) are tightened on their own;
- inflammatory diseases (bronchi, lungs) - begin due to infection with the introduction of the instrument. In such cases, antibacterial therapy is carried out.
