General information

 

There are three types of acoumetry for examining the hearing threshold in young children:

  • computer - an objective method that is used for all age categories, including newborns;
  • behavioral - designed for ages 4 to 36 months;
  • with visual reinforcement - designed for ages 12-36 months;
  • play - designed for children aged 3 to 5 years.

For older children, adult methods can be used:

  • speech - often used to correctly select hearing aids;
  • tonal - studies the threshold of hearing sensitivity to waves with different frequencies. Allows to determine the maximum discomfort limit, which is the limit for the subject.
Hearing acuity for tone purity is carried out by three methods: Weber, Rinne and Schwabach using a tuning fork.

Procedure

Preparation

Does not require special preparation.

Conducting

Computer. No active participation is required during this manipulation. The entire process occurs automatically, during the sleep of the person to whom the sound is fed into the ear. Electrodes are attached to the head of the subject, with the help of which the brain activity is recorded in response to audible messages. The result is output as an audiogram to a connected computer system.

Behavioral. The child is placed in a soundproof booth with an adult in such a way that the speakers are placed on both sides. The specialist begins the examination with 30 dB HL. Any reactions of the child to the voice are noted. Usually familiar words are pronounced: names, favorite titles, etc. To find out which ear hears better, the slightest manifestations in the direction of one of the speakers are noted.

With visual reinforcement. The child is positioned in the same way as for the behavioral examination. Only mechanical play devices are placed under or above the speakers, which move or light up with lights. Sometimes a monitor with moving pictures is placed in front of the patient. A "barking" signal with a frequency of 1000 Hz and an intensity of 50 dB HL is heard in the free space. If the baby reacts to it, the toys start moving. If there is no reaction, the tone intensity increases until the patient responds. The examination is repeated at frequencies of 250, 2000, 4000, 500 Hz. During it, it is necessary to achieve a result when the child will combine an audible push with the beginning of the movement of visual moments (pictures or toys). The reaction is considered a response from the better hearing ear.

Play. The patient must perform the agreed action (for example, put a ring on a pencil or pick up a toy) at the moment the signal appears. At the beginning of the manipulation, a sound of 1000 Hz with an intensity between 30 and 40 dB IF is given. The procedure for studying the threshold of perception is the same as in traditional acumetry. Sound audibility is determined at 250-4000 Hz for each ear separately.

Speech. The ear that is not being tested is covered with cotton wool or another device. Certain words are pronounced from different distances, which are recorded on paper. The hearing limit determines the acuity of perception of whispered and normal speech.

The probability of lip reading is not allowed. It should be known that the result of the study depends on the richness of the vocabulary and the level of development of the person, the complexity of the pronounced phrases, therefore, a unified set of words is pronounced during the procedure.

Tone. A special device - an audiometer - gives a sound signal through headphones worn by the patient. The result should be an instant pressing of the button. The test results are recorded on an audiogram, which is later deciphered by a specialist.

Rehabilitation period

There is no rehabilitation period.

Indications

Audiometry is necessary for a child in the following cases:

  • frequent otitis, inflammatory ear diseases;
  • hearing loss;
  • choosing a hearing aid;
  • testing hearing in newborns;
  • assessing the correctness of treatment.

Main contraindications

There are no contraindications for audiometry. The technique does not require injections or administration of drugs into the ears. Takes from 10 to 30 minutes.

Complications

Due to the absolute simplicity of the examination, no complications were recorded.
