General information
Fractures in the skeletal structures can be open or closed. Open fractures are diagnosed visually by the exit of fragments. With closed ones, everything is much more complicated. Pathology is often accompanied by damage to ligaments, muscle fibers, blood vessels, nerves. Therefore, during an appointment, the doctor must palpate the pulse distally from the site of deformation and check the functioning of nerve cells.
The process
Diagnostics of bone fractures begins with identifying the possibility of active or passive movement of the damaged part of the body. Additional data can be obtained by measuring the volume and length of the injured section. In case of fractures, the injured arm or leg is shortened due to deformation in the focus of pathology.
X-ray diagnostics is the main method of examination. X-rays are performed after local anesthesia. The images show the injury in two mutually perpendicular projections.
During an X-ray examination, the condition of the soft tissues in the area of damage is also analyzed, since a hematoma may form due to bone fragments. In the image, it will appear as a dark spot with uneven contours.
Control X-rays are taken after applying a plaster cast to check for complications.
An X-ray can be taken with portable equipment if the patient's condition does not allow this to be done in the X-ray room.
Based on the image on the film, the course of the damage line is judged; according to this data, pathological changes in the bones are divided into transverse, longitudinal, oblique, comminuted, T-shaped and Y-shaped. The image also determines the nature of the displacement.
Fracture diagnostics are currently carried out by other means: computed tomography and magnetic resonance imaging, ultrasound examinations, scintigraphy, sonography. These studies are auxiliary, they are used to determine pathologies in soft tissues located near the fragments, as well as to clarify the structure of bone tissue. In addition, such measures are used in cases where X-rays are contraindicated.
Indications
A check for a bone integrity violation should be carried out with the following signs:
- change in limb length;
- abnormal mobility in the lesion;crunching sound that can be determined by ear;
- worsening pain when palpating;
- edema and swelling;
- internal hemorrhage.
Nerve trunk ruptures can be suspected with the following signs:
- lack of sensitivity in the damaged part of the body;
- limited mobility limbs.
Symptoms of damage to large blood vessels include:
- a decrease in temperature of the area below the injury;
- no pulse below the site of pathology;
- paleness of the skin;
- sagging veins located near the site of pain.
Contraindications
X-ray irradiation is strictly contraindicated for pregnant women on the spine and ribs, in the pelvic area, and is not recommended on the arms or legs.
In addition, X-rays cannot be prescribed to people with thyroid disease and with increased sensitivity to iodine preparations, patients with active tuberculosis, with heart rhythm disturbances in patients, hypotensive and hypertensive patients, with a tendency to form blood clots.
