Gonioscopy is an ophthalmological test that allows for a precise examination of the anterior chamber of the eye. Typically, this chamber is located between the iris and the cornea.

General information

Before starting microgoniometry, the specialist must adjust the lighting and optical parts of the lamp. The base of the gonioscope, through which the angle of the anterior chamber is examined, is wiped with chamois leather, and the corneal and scleral parts are disinfected with wet wipes. They are moistened in mercury oxycyanide.

The following gonioscopes are used today:

  • single-mirror;
  • three-mirror;
  • four-mirror.
When the ophthalmologist examines the upper and lower sections of the angle, the illuminator is on the right side at an angle of 15–30°. In order to examine the lateral sections, the device is installed on the opposite side of the angle image.

Methods and technologies

When examining different sections of the angle, the illuminator and microscope must be moved taking into account the position of the surface of the device. The anterior chamber angle can be easily distinguished if there is tear fluid in the capillary space between the cornea and the surface. This angle is examined using different lighting methods used during biomicroscopy.

To obtain detailed gonioscopy, the procedure is performed in direct illumination using a slit. As a rule, this can be used to obtain:

  • reduction in the angle of biomicroscopy;
  • cutting out an optical section of the angle.

In order to identify pathological changes in the angle area, to conduct differential diagnostics between a formation and a root cyst, it is recommended to use indirect or diaphanoscopic light. The anterior chamber angle is examined under any magnification of the microscope. It is best to use 16-20x magnification.

Procedure

Preparation

Before the procedure, the patient is given eye drops (during this, the eye is instilled with dicaine solution 3 times). After this, the patient sits in front of the slit lamp, and the doctor fixes his head on a special stand. Then the focus of the illuminator and microscope is aimed at the patient's cornea. This focusing significantly facilitates microgonioscopy.

Process of gonioscopy

After opening the slit of the eye, the patient should look down first, and then up. Then the doctor inserts the gonioscope into the conjunctival cavity. In this case, the body of the device must be held with the left hand, and the illuminator and microscope of the slit lamp must be controlled with the right hand. The lower parts of the eye are examined first.

As a rule, the examination is carried out in diffuse lighting. A gonioscopic attachment is used, which is put on the prism of the illuminator, due to which it evenly illuminates a circle with a diameter of about 20 mm.

As soon as the procedure is completed, the patient looks up, and the ophthalmologist pulls the lower eyelid down with a finger. Then the lower half of the scleral ring, as well as the gonioscope, are removed from the conjunctival cavity. And the corneal-scleral part of the device is wiped with tampons that are moistened in a solution of mercury oxycyanide. This is done in order to remove mucus from it. Sodium albucid is instilled into the patient's conjunctival cavity.

The entire process lasts about 15 minutes. In addition, gonioscopy is absolutely painless.

Rehabilitation period

After gonioscopy, the patient is prescribed drops with an antibacterial effect to prevent infections.

Indications

Usually, gonioscopy of the eye is performed in the following conditions:

  • glaucoma;
  • injuries in the angle of the chamber;
  • congenital anomalies in the development of the chamber;
  • vascular diseases of the eye;
  • oncological diseases.

In addition, gonioscopy is performed in order to:

  • monitor treatment glaucoma;
  • to detect foreign bodies that impede the outflow of moisture.

Contraindications

Gonioscopy is not allowed if the patient has the following conditions:

  • purulent eye disease;
  • alcohol or drug intoxication is observed;
  • the integrity of the eye is compromised due to injury;
  • the presence of a mental disorder.

The patient should not have contact lenses at the time of the eye examination.
