General information
During this operation, which is often performed for chronic pancreatitis, the dissected pancreas and jejunum are anastomosed to further ensure free outflow pancreatic secretion from small ducts. Procedure
Preparation
Preparation for surgery involves the patient undergoing a comprehensive examination to determine the scope of surgical intervention:
- patient history;
- laboratory tests;
- radiology of the abdominal cavity, stomach and duodenum;
- computed tomography of the pancreas;
- retrograde endoscopic cholangiopancreatography (to determine the diameter of the pancreatic duct).
Procedure pancreatojejunostomy
The intervention is performed through a wide midline laparotomy (abdominal wall incision):
- the gastrocolic ligament is intersected and the stomach is retracted upwards;
- the gland duct is punctured using a syringe, through the needle of which the contents are collected for subsequent analysis;
- the parenchyma is dissected, the lumen above the duct is opened using a scalpel;
- a loop of small intestine is formed and passed into the omental bursa through the mesentery of the transverse colon;
- the antimesenteric edge of the intestine is incised and sutures are placed through the lower part of the duct, through the thickness of the gland itself and through the upper part of the enterotomy opening;
- the sutures are held with special clamps and part of the loop of thin intestines;
- the lower sutures are tied and their ends are cut off (the same is done with the upper row of sutures);
- the intestinal loop is sewn to the mesentery of the transverse colon and drainage is performed with the drainage being brought to the anterior lower edge of the pancreas and brought out through an additional incision;
- the abdominal wall is sutured.
The operation is performed strictly under general anesthesia, which is administered to the patient intravenously.
