General information

Hydrocele (dropsy of the testicle) is a pathological condition in which fluid accumulates in the membranes of the testicle. This disease mainly affects boys, but in some cases it is diagnosed in adult men. It manifests itself as a painless enlargement of the scrotum on one or both sides.

Causes of hydrocele:

  • non-closure of the vaginal process of the peritoneum;
  • excessive secretion of serous fluid by the membranes of the testicle;
  • tumor formations of the scrotum and its organs;
  • inflammatory processes of the testicle and (or) its appendages;
  • filariasis;
  • scrotum injuries;

Classification is carried out based on genesis:

  • congenital hydrocele;
  • acquired hydrocele.

Depending on the ongoing processes:

  • congenital;
  • acquired.

By localization of the scrotum lesion:

  • unilateral;
  • bilateral.
In newborn boys, hydrocele of the testicle develops only in the case of communication of the vaginal process of the peritoneum with the abdominal cavity, in older children and adult men, the causes of hydrocele are completely different.

Surgical operations for hydrocele are divided into:

  • radical (operation according to the Bergman, Lord, Winkelmann method);
  • sparing (puncture, sclerotherapy).

In case of congenital hydrocele, the operation is performed according to the method Ross.

The process of the operation

Preparation

Before the surgery, the patient undergoes a physical examination (palpation of the scrotum) by the attending physician, an anesthesiologist consultation, and laboratory blood and urine tests, as well as additional examination methods:

  • Ultrasound;
  • diaphanoscopy.

The process of hydrocele surgery

The process of the operation depends on the chosen method:

  • According to Winkelmann: dissection of one of the proper membranes of the testicle along the anterior surface, turning it inside out and suturing it behind the testicle.
  • According to Bergman: dissection of the membranes of the testicle and removal of part of the inner leaflet with subsequent suturing of the remaining part.
  • According to Lord: dissection of the testicular membranes with their subsequent corrugation.
  • According to Ross: dissection of the anterior wall of the inguinal canal to provide access to the vaginal process of the peritoneum. The appendix is ligated near the internal ring of the inguinal canal.
  • Puncture: a puncture needle is used to puncture the testicular membrane and suck out the accumulated fluid.
  • Sclerotherapy: evacuation of serous fluid from the testicular membranes with subsequent introduction of sclerosing agents into them, which cause a non-infectious inflammatory process and fusion of the testicular membranes - the cavity for fluid accumulation disappears.

Rehabilitation period

After the operation, it is recommended:

  • taking antibacterial and painkillers;
  • excluding sexual activity, lifting weights and playing sports for a period of time recommended by the attending physician;
  • to reduce the risk of premature destruction of surgical sutures, take a shower for the first time, rather than bath;
  • wearing a support bandage for two weeks - a suspensory or special underwear.

Indications

This operation is performed in cases where there is a tight, increasing increase in one or both halves of the scrotum.

A sign of an acute form of hydrocele is sharp pain and swelling in the scrotum.

Contraindications

This operation is contraindicated in the following cases:

  • chronic diseases in the stage of decompensation;
  • blood clotting disorders.

Complications

During this procedure, the following complications may occur:

  • bleeding;
  • hematoma formation with subsequent suppuration;
