General information
A disease in which the testicle (or testicles) do not descend into the scrotum is called cryptorchidism. In this case, the testicle can be located either in the abdominal cavity or in the inguinal canal. The operation to lower the testicles is performed to achieve the following results:
- prevention of malignancy;
- preservation of hormonal functions and fertility;
- achievement of a cosmetic effect;
The optimal age for the operation is considered to be the period from 5-6 to 18 months, since performing orchiopexy in this period allows maximal preservation of the hormonal functions of the organ and spermatogenesis, as well as reducing the risk of tumor neoplasms.
There are several methods for performing this operation, which are divided into:
- one-stage (the testicle descends into the scrotum and is fixed to its wall - the Petrivalsky method);
- two-stage (the testicle after lowering is fixed to the fascia hips - laparoscopic method)

Preparation
Before the surgery, a thorough physical examination of the patient is performed, as well as the appointment of appropriate laboratory tests of blood and urine. Since the operation is performed under general anesthesia, an examination and consultation with an anesthesiologist is carried out.
Process of orchiectomy. Procedure for lowering the testicles
During a one-stage operation, an incision is made in the inguinal canal: the skin and muscles are dissected layer by layer and the inguinal canal itself is opened. An undescended testicle is detected in the inguinal canal. After this, an incision is made in the scrotum and, using a clamp, the testicle is lowered into the scrotum through the external inguinal ring, where it is fixed to its wall. The wounds are sutured. The inguinal canal is sutured with plastic surgery of the wall.
If the operation is performed on a patient who has not reached the age of majority, then written permission from the parents is required to perform such a manipulation, the use of anesthesia and possible removal of the retained testicle.
In the case of a high location of the testicle in the abdominal cavity and the impossibility of lowering it in one stage, a two-stage laparoscopic method is used. The first stage is the intersection of the testicular vessels and maintaining the blood supply to the testicle due to the vessels of the vas deferens. The second stage of the operation is carried out after 6 months, as a result of which the testicle with the vas deferens is laparoscopically isolated on a wide sheet of peritoneum and lowered into the scrotum, where it is fixed to its wall. The wounds are sutured.
Rehabilitation period
During the rehabilitation regimen, it is recommended:
- taking antibacterial and painkillers;
- bed rest for 2-4 days after the operation;
- excluding physical activity in the next 2 weeks after the operation.
The stitches are removed on the 7th-10th day.
The patient is discharged on the 8-10th day after the operation.
Indications
An indication for orchiopexy (lowering) is undescended testicle or testicles.
Contraindications
Contraindications to surgery are:
- severe concomitant diseases in the decompensation stage;
- blood clotting disorders.
Complications
As a result of this procedure, there is a possibility of the following complications:
- bleeding;
- scrotal edema;
- damage to the vas deferens;
- inflammatory processes of the testicle and appendages;
- shrinkage and atrophy of the operated testicle;
- repeated slippage of the testicle from scrotum;
- postoperative wound infections.
