General information

Microdiscectomy is a neurosurgical operation, the technique of which is similar to discectomy. The difference between them is that in this situation, a minor surgical intervention of high precision occurs, which allows avoiding many complications. Thanks to the use of high-precision equipment (surgical microscope) during microdiscectomy, recovery occurs much earlier.

Most often, microdiscectomy of the lumbar region is performed.

The technique of the operation involves the implementation of all necessary surgical manipulations through a small incision. With the help of a microscope, the doctor performs precise actions with microsurgical instruments that do not affect normal tissues and muscles. As a result, it is possible to remove the intervertebral hernia, the nucleus pulposus in full, without violating the integrity of the bone structures. After the hernia has been removed, laser restorative irradiation of the intervertebral discs is performed.

Preparation

Preparation consists of the following:

  • blood tests;
  • discography, magnetic resonance imaging.

A week before the scheduled surgery, you must stop taking the following medications:

  • anti-inflammatory drugs (aspirin);
  • blood thinners (warfarin, clopidogrel).

Only light food can be eaten in the evening before the procedure. It is forbidden to eat and drink after midnight.

Microdiscectomy procedure

Microdiscectomy (cervical, lumbar and sacral) is performed in several ways:

  • Endoscopic - the skin incision is about 3 cm. An endoscope is inserted through it, with the help of which the hernia is removed.
  • Percutaneous - the hernia is removed through a cannula. The intervention is performed using a stylet under local anesthesia.
  • Laser - evaporation (vaporization) of the nucleus pulposus of the intervertebral disc is performed using a laser pulse.
  • Percutaneous vertebropulpous extraction - is performed using a fluoroscope.

The operation is performed under general anesthesia. Local anesthesia is also possible. Mandatory disinfection of the skin area at the site of the future incision is carried out. Only after this, the specialist makes an incision and carries out the appropriate surgical manipulations. They depend on the type of microdiscectomy performed. Upon completion of all necessary manipulations, the doctor performs layer-by-layer suturing of the surgical wound.

The duration of the operation directly depends on the method of its implementation. The duration can be from 30 minutes to 1.5 hours.

Rehabilitation period

  • pronounced manifestations of radicular compression;
  • the presence of a large hernia;
  • ineffectiveness of drug treatment;
  • compression of blood vessels and nerves;
  • muscle weakness;
  • impaired urination.

Contraindications

This procedure is not performed in cases:

  • spinal cord injury;
  • infectious lesions of the spine;
  • oncology, stenosis;
  • neurological disorders;
  • degenerative changes in the spinal disc.

Complications

The following consequences are possible:

  • rupture of the spinal membrane;
  • damage to the nerve root;
  • fecal/urinary incontinence;
  • bleeding;
  • development of infections.
