General information

Orchiectomy is performed to reduce testosterone levels in a man's body (if a testicular tumor has been diagnosed) or to prevent cancer. Bilateral orchiectomy is performed as one of the stages of the patient's sex change surgery.

Procedure

Preparation

A thorough medical examination of the patient is performed before the surgery. The appropriate laboratory blood and urine tests are prescribed, as well as, if necessary, additional research methods for the most accurate diagnosis.

A week before the surgery, you should stop taking medications containing aspirin, and two to three days before - any non-steroidal anti-inflammatory drugs.

Process of orchiectomy

There are several types of orchiectomy:

  • Simple orchiectomy: an incision is made in the middle of the scrotum, through which the testicle and part of the spermatic cord are removed. The wound is sutured with two rows of stitches, after which a sterile dressing is applied.
  • Subcapsular orchiectomy: is performed according to the principle of simple orchiectomy, but in this operation only the glandular tissue from the shell of each testicle is removed and the appearance of the scrotum is preserved.
  • Inguinal orchiectomy: an incision is made in the groin area, after which the testicle is removed together with the spermatic cord, the surgical field is washed with sterile saline and the wound is sutured layer by layer, followed by the application of a sterile dressing.
During the operation, at the patient's request, a testicular prosthesis can be installed in the scrotum, which will preserve the appearance of the organ. The same procedure can be performed some time after orchiectomy.

Rehabilitation period

After this operation, the patient is recommended to:

  • take antibacterial and painkillers;
  • drink plenty of fluids for several days after the operation, with the exception of drinks containing caffeine and alcohol;
  • avoid sexual activity, lifting weights and playing sports for the period of time recommended by the attending physician;
  • to reduce the risk of premature failure of surgical sutures, take a shower for a week instead of a bath;
  • wear a special support bandage - a suspensory for two weeks after the orchiectomy;

Indications

Orchiectomy is indicated for:

  • severe traumatic injuries (crushing, rupture, detachment of the testicle from the spermatic cord);
  • infectious process in the scrotum, which is accompanied by extensive damage to the tissues of the testicle and its appendages, tuberculosis, acute orchitis);
  • testicular cancer, prostate cancer;
  • tumor formations;
  • sex change.

Contraindications

Orchiectomy is contraindicated for:

  • chronic diseases in the stage of decompensation;
  • mental disorders;
  • violation blood clotting.

Complications

During this procedure, the following complications may occur:

  • bleeding;
  • formation of hematomas of the scrotum and, as a result, its suppuration;
  • infections.

If orchiectomy is performed under epidural anesthesia, then the following are possible:

  • bleeding into the spinal canal;
  • damage to the spinal nerves;
  • post-puncture headache.
