General Information
Compared to estradiol and estrone, estriol has the lowest proportion. It prepares the uterus for embryo growth and childbirth, enhancing blood flow to uterine tissues and vessels and activating mammary duct development for breastfeeding.
This test is highly informative, allowing obstetricians to assess placenta and fetal health. In men, estriol levels remain stable at ≥ 0.07 ng/mL throughout life. In women, it is < 0.25 ng/mL but rises significantly from the sixth week of pregnancy. Elevated levels may indicate liver pathology in both sexes.
A sharp rise above normal suggests a risk of miscarriage or preterm birth in late pregnancy.
Low estriol may follow use of penicillin, ampicillin, or tromethamine. In pregnant women, it may indicate fetoplacental insufficiency, infections, or fetal defects like heart anomalies, Down syndrome, adrenal hypoplasia, or anencephaly.
Indications for Free Estriol Test
An obstetrician-gynecologist or endocrinologist orders this test to evaluate placenta condition, fetal disease risks, developmental anomalies, or late toxemia.
It is also indicated for diagnosing hormonally active testicular, ovarian, or adrenal tumors and suspected hyperestrogenism.
How Free Estriol Test Is Conducted
The last meal is 8 hours prior; non-carbonated water is allowed. Avoid heavy exercise for an hour before. Rest for 30 minutes pre-test. Venous blood is drawn in the morning on an empty stomach. The patient sits, a tourniquet is applied, and a syringe collects the sample.
Peak free estriol levels occur between 2:00 PM and 3:00 PM.
