General Information

Estradiol is produced by the ovarian follicular apparatus in women, testes in men, and, to a lesser extent, the adrenal cortex in both sexes.

Known as a female hormone, it is linked to the development and function of the female reproductive system and overall body, influencing:

  • menstrual cycle and ovulation;
  • secondary sexual characteristics;
  • reproductive system development (in adolescence) and uterine growth during pregnancy;
  • emotional behavior;
  • subcutaneous fat deposition on hips and abdomen.

For both men and women, it affects:

  • bone maturation in adolescence and metabolic acceleration;
  • increased blood clotting;
  • cholesterol reduction.
Estradiol has pronounced anabolic properties, and high concentrations may cause irritability and nervous tension.

It helps retain sodium and water in the body at necessary levels.

In men, estradiol levels remain stable (7.63–42.6 pg/mL), while in women, they fluctuate with the cycle:

  • Days 1–6 (menstrual phase) and 3–14 (proliferative phase): 12.5–166;
  • Days 13–15 (ovulatory): 85.8–498;
  • Day 15+ (luteal): 43.8–211;
  • Postmenopause: ≤ 54.7;
  • First trimester of pregnancy: 215–4300.

Elevated levels may result from hypopituitarism, Turner syndrome, Stein-Leventhal syndrome, amenorrhea, postmenopause, or intense physical exertion.

Low levels may indicate early puberty, hyperthyroidism, gynecomastia, cirrhosis, or ovarian/testicular tumors.

Indications for Estradiol Blood Test

A general practitioner, gynecologist, reproductive specialist, endocrinologist, or oncologist may order this test for:

  • Menopausal symptoms (hot flashes, sweating, insomnia);
  • Abnormal (slowed/accelerated) genital development;
  • Pelvic pain or abnormal vaginal bleeding;
  • Menstrual irregularities or fertilization difficulties;
  • Monitoring infertility treatment (follicle growth);
  • Diagnosing testicular, ovarian, adrenal, or breast tumors (in men).

How Estradiol Blood Test Is Conducted

Blood is drawn from a vein in the morning on an empty stomach. The patient sits, a tourniquet is applied, and a syringe collects the sample. The puncture site is disinfected, and the arm bent at the elbow.

The last meal should be 8 hours prior; water is allowed. Discontinue medications 10–14 days before, or list them on the form. Avoid fatty/fried foods, alcohol, and smoking 24 hours prior.

Women should test on days 3–5 or 19–21 of their cycle.

Completion Time for Estradiol Blood Test

Results are available within 1–3 hours, though labs typically provide them the next day.
