General Information

Transudate is edematous fluid that gradually accumulates in the body, often due to disruptions in the lymphatic or circulatory systems or serum leakage. It may also result from imbalances in salt and water metabolism. Transudate does not cause inflammation or tissue transformation, and individuals are often unaware of its progressive condition.

Exudate is a watery fluid leaking from small blood vessels, appearing cloudy and containing high levels of albumin, hematogenous and gestagenic amino acid chains, erythrocytes, and microbes.

Serous fluid is a clear proteinaceous fluid secreted by serous membranes. Excreta are metabolic byproducts, and their excretion is disrupted when bodily systems malfunction.

Indications for Cytology of Transudates, Exudates, Secretions, and Excreta

Experts recommend cytology for suspected:

  • transudate in serous membranes;
  • secreting breast (discharge);
  • bladder neoplasms;
  • non-tumorous or pre-tumorous conditions.

How Cytology of Transudates, Exudates, Secretions, and Excreta Is Conducted

Exudates and excreta: Biomaterial is placed in a sterile container and immediately analyzed to prevent cell breakdown.

The type of pleurisy, chylothorax, hemothorax, pericarditis (fibrinous, exudative), or peritonitis is determined. Exudate is characterized by a relative density above 0.18, fibrinogen content over 30 g/L, LDH activity of 1.6 mmol/L per hour, and no glycemia.

Transudates: Dead tissue is removed from the puncture site, followed by pleural or peritoneal puncture. Up to one liter of fluid is collected in a sealed container.

Breast secretions: Smears are made by pressing a clean slide against the nipple, collecting the first and subsequent drops, spreading them thinly in the center, and allowing them to dry.

The procedure is performed in the morning on an empty stomach without special diets. Lab analysis must occur the same day to avoid sample degradation.

Biomaterial is labeled and sent for further testing, ideally repeated at least three times.
