General Information

The protein responsible for hemostasis is fibrinogen, the foundation of a blood clot (thrombus).

Thrombus formation occurs as follows: fibrinogen transforms into insoluble fibrin, forming threads that weave into a fibrin mesh. Alongside platelets, this seals the bleeding site until the vessel heals, enabled by proper coagulation factor function.

Indications for Fibrinogen Testing

Elevated fibrinogen levels may indicate conditions like:

  • Myocardial infarction;
  • Stroke;
  • Amyloidosis;
  • Malignant tumors.

Levels rise post-burns, with oral contraceptives or estrogens, or after surgeries.

Fibrinogen increases during inflammation, tissue necrosis, and pregnancy.

Reduced levels signal heart failure. As fibrinogen is produced by liver cells, a decrease may indicate liver tissue damage.

Low levels also risk significant blood loss from minor skin injuries.

Pre-surgery, fibrinogen testing is mandatory to avoid complications.

How Is the Fibrinogen Test Conducted?

The test measures thrombin clotting time in diluted plasma using venous blood.

Plasma is extracted by mixing the sample with sodium citrate and centrifuging.

Testing occurs mornings (8–12 AM) on an empty stomach, with only water allowed.

Preparation rules:

  • Last meal 8–12 hours prior;
  • Avoid fatty, fried food, alcohol, and smoking 24 hours before;
  • Ideally test before starting medications;
  • Avoid emotional stress 30 minutes prior;
  • No physical exertion hours before.

Factors affecting results:

  • Medications like estrogens, contraceptives, anabolic hormones, androgens, phenobarbitals, or anticoagulants;
  • Improper blood collection/mixing, causing clots and disrupting blood-to-citrate ratios.
