Platelets are formed, non-nucleated blood elements produced by bone marrow cells (megakaryocytes). Their primary function is participation in all phases of blood clotting. They serve as the building material for any primary thrombus, releasing substances necessary for clot consolidation. Platelets are essential for strengthening blood vessel walls and maintaining their structure. Normally, the platelet count (PLT) in 1 microliter (µL) of human blood ranges from 180,000 to 400,000. Changes in this indicator often serve as a sign of physiological process disruptions or the detection of pathological changes in the body.

Indications for Platelet Counting

An increase in the number of these blood cells (thrombocytosis) may result from nervous system excitation after physical activity.

In other cases, it indicates a pathological condition:

  • Asphyxia or burns;
  • Hemolytic crisis and consequences of bleeding;
  • Splenectomy;
  • Malignant neoplasms;
  • Rheumatoid arthritis and ulcerative colitis;
  • Muscle trauma;
  • Various forms of leukemia and tuberculosis.

In such cases, their count in 1 µL can reach millions.

The reverse process – a decrease in number (thrombocytopenia) – occurs with reduced megakaryocyte productivity. Falling below the critical minimum of 50,000/µL may lead to the development of hemorrhagic syndrome. In some women, platelet counts decrease during menstruation.

Main pathologies accompanied by thrombocytopenia:

  • Various severe intoxications and viral infections;
  • Infectious-toxic and toxic-allergic conditions (chemical and food allergens);
  • Aplastic anemia;
  • Radiation exposure;
  • Liver diseases;
  • Acute leukemia.

How Platelet Counting Is Conducted

The requirements for sample collection for this test are identical to most hematological tests: morning collection of venous or capillary blood after a 12-hour diet, excluding pharmacological drugs and any physical activity.

The accuracy and reliability of the test largely depend on the correct venous blood collection technique.

Currently, three main platelet counting methods are predominantly used in laboratory practice:

  • Counting using an immersion microscope system – in stained blood smears.
  • Microscopic counting in a Goryaev chamber with prior preparation of a liquid mixture.
  • Mixing the biosample with reagents and processing data with automated hem analyzers.

The first two methods require appropriate staff qualifications and are performed manually. The automated method is less labor-intensive but requires compliance with material preparation and equipment operation rules.

If manual methods are used, results are ready within 2-3 hours. Depending on the model, hematology analyzers can reduce the full analysis time to 90-180 seconds. One measurement cycle takes less than a minute.
