General Information
The primary lactate source in plasma is red blood cells. During exercise or exertion, it exits muscle tissues, converting to pyruvate or being metabolized by the heart and brain epithelium.
Blood lactate is a key enzyme formed during normal glucose processing. With metabolic or oxygen delivery disruptions, lactic acid salts rapidly accumulate, a condition called lactic acidosis, divided into types A and B:
- Type A (oxygen supply-to-tissue disorders);
- Type B (excess production or insufficient utilization).
Elevated LA levels appear before other indicators (oliguria, hypotension, pH decline).
With adequate oxygen, pyruvate metabolizes into water and carbon dioxide; with insufficient oxygen, it converts to lactic acid.
In healthy individuals, the plasma pyruvate/lactate ratio averages 10 (9.3–14.3), and changes indicate metabolic disruptions.
Indications for Blood Lactate Testing
This test is widely used in sports medicine and physiology to assess an athlete’s fitness, exercise intensity, and effectiveness.
It is also indicated for:
- Type 2 diabetes;
- Alcoholism;
- Hepatitis;
- Hemorrhagic shock;
- Left ventricular failure;
- Cirrhosis;
- Anemia;
- Leukemia;
- Myocardial insufficiency;
- Pancreatitis;
- Pyelonephritis;
- Reye’s syndrome;
- Neonatal asphyxia.
Normal blood LA salt levels should not fall below 1 mmol/L. Testing is recommended for diagnosing tumors and determining acidosis causes.
Lactic acidosis may also occur after high doses of ethanol, aspirin, insulin, adrenaline, fructose, or sorbitol.
How Blood Lactate Testing Is Conducted
A therapist, surgeon, infectious disease specialist, or resuscitator orders the test. A kinetic colorimetric method is used (based on reaction rate dependence on substance concentration).
The patient must rest (lie down for 1 hour) and avoid food for 8-10 hours prior.
Post-venipuncture, the sample is placed in a tube with sodium fluoride. No tourniquet is used, and the puncture site is pressed with a cotton swab until bleeding stops.
