This is creatine anhydride, one of the end products of nitrogen metabolism, formed by the cleavage of a phosphoric acid residue from creatine phosphate. It is a critical metabolite in muscle energy metabolism, excreted in urine. Its blood level depends on a person’s total muscle mass and kidney excretory function. With fully functioning kidneys, creatinine is completely excreted daily. Disruptions in this continuous mechanism increase its serum levels.

Indications

Possible reasons for elevated or reduced creatinine include:

  • Acute and chronic renal failure from prerenal (shock states), renal (parenchymal diseases), or postrenal (urinary tract obstruction) factors;
  • Glomerulonephritis, tubulointerstitial nephritis, kidney damage from systemic diseases, vasculitides, Goodpasture syndrome, tumors, or kidney stones;
  • Nephrotoxic substance effects (ethylene glycol, methanol, mercury, copper, chromium, aminoglycosides, sulfonamides, cyclosporine, mushroom or insect/snake venom poisoning);
  • Diabetes mellitus (diabetic nephropathy);
  • Muscle mass reduction, muscular dystrophy, polymyositis;
  • Pregnancy;
  • Anemia, leukemia;
  • Hypothyroidism, hypercortisolism, acromegaly;
  • Starvation.
Normal blood creatinine levels: men – 53-106.1 µmol/L, women – 44-97.2 µmol/L.

How It Is Conducted

A healthcare worker sterilizes the cubital vein puncture site, secures a tourniquet mid-arm, and draws venous blood with a sterile needle, then releases the tourniquet. The puncture is sealed with an antiseptic plaster.

Urine is collected over 24 hours, skipping the first morning void. The Zemnietsky test is used, collecting urine in a container (minimum 5 liters) throughout the day.

For accurate molecular readings, avoid sports for two days prior, limit diet for 24 hours (no meat, protein-rich foods, coffee, tea, or carbonated drinks), and drink sufficient water.

For suspected nephritogenic conditions, hemorenal tests are performed.
