Total bilirubin does not exist in pure form but is determined by the formula: indirect (highly fat-soluble, formed from hemoglobin breakdown) + direct (formed in the liver, water-soluble).
Indications for Total Bilirubin Testing

The test is conducted in cases such as:

How Total Bilirubin Testing Is Conducted
A specialist collects venous (or rarely capillary) blood. A colorimetric (photometric) method is used. Avoid food intake for 12 hours before the test, exclude physical exertion, and refrain from smoking and alcohol.
Reference values for men and women: 8.5-20.5 mmol/L.
Bile pigment accumulates excessively with red blood cell destruction, liver failure, or bile duct obstruction. Levels rise in hemolytic diseases, including oncological processes and infections, hepatocyte damage (inflammatory, toxic, or tumor-related), intra- or extrahepatic duct obstruction, and biliary tract anomalies.
Levels exceed the norm with exposure to toxic hepatotropic or cholestatic drugs, jaundice in pregnancy, fructose intolerance, galactosemia, and syndromes like Dubin-Johnson, Crigler-Najjar, Gilbert, and Rotor.
In newborns, physiological jaundice occurs due to increased red blood cell breakdown with an underdeveloped bilirubin-conjugating system.
