What is the Urine Amylase (Diastase) Test
This calcium-dependent enzyme is excreted by the kidneys in urine. A decrease or increase in its level helps diagnose gland dysfunction caused by various pathologies and determine the etiology of abdominal pain.
The test may reveal levels below or above the norm.
A level below 16 may indicate liver inflammation, either early-stage or chronic exacerbation, and is common in pregnant women with preeclampsia.
Elevated levels have various causes:
- Slight deviation suggests acute inflammatory infections in abdominal organs (e.g., appendicitis, gastritis, peritonitis, colitis).
- In pancreatitis and other pancreatic tissue inflammation, amylase reaches 120-260 units.
- Levels of 160-640 (tenfold excess) may indicate kidney failure, diabetes, or cholecystitis.

Ordered by a general practitioner, pediatrician, family doctor, nephrologist, obstetrician-gynecologist, or surgeon to diagnose:
- Acute viral infections;
- Chronic or acute pancreatitis;
- Liver cirrhosis;
- Hepatitis;
- Peritonitis, appendicitis;
- Diabetic acidosis;
- Intestinal obstruction;
- Stomach cancer;
- Alcohol, drug, or other intoxication;
- Pancreatic tumors, atrophy, or necrosis.
It’s also performed post-surgery for cardiovascular conditions, prolonged medication or narcotic use, and during pregnancy.
How the Urine Amylase (Diastase) Test Is Performed
Preparation
For 24 hours before collection, avoid antibiotics, analgesics, NSAIDs, and adrenaline-boosting drugs.
Collection
Collect 100-150 mL of morning urine after washing external genitals with warm water (no soap). Use a pharmacy-purchased sterile container or a clean, dry 200 mL glass jar with a tight lid.
The biomaterial must be delivered to the lab immediately, ideally tested at the medical facility.
