What is AST (Aspartate Aminotransferase)
Aspartate Aminotransferase (AST), formerly glutamate oxaloacetate transaminase (GOT), is a transaminase enzyme catalyzing amino group transfers in the body. Found in hepatocyte cytoplasm and mitochondria, it’s also in kidneys, myocardium, pancreas, spleen, skeletal muscles, and brain tissues, supporting carbohydrate-protein metabolism.
Indications for AST Testing
AST detection in serum isn’t liver-specific. It’s used alongside other markers (ALT, alkaline phosphatase) to identify pathology causes.
Elevated GOT is typical in some hepatitis forms, acute myocardial infarction, biliary obstruction, sepsis, and cancers.
Reduced levels occur with chronic renal failure, azotemia, alcohol exposure, and certain medications.
How AST Testing Is Performed
Venous blood is drawn, with serum or plasma separated within 2 hours, free of hemolysis.
Measurements use reagent kits manually or with analyzers. Reagents mixed with the sample assess AST activity via NADH reduction rate (optical density), proportional to AST concentration, measured at 340 nm photometrically. Results are calculated by formulas.
Reference AST range at 37°C (U/L): men ≤ 37, women ≤ 31.
Newborns and infants typically have higher levels than adults. Data are indicative; serial tests every 7 days within one lab are most significant.
Analytical sensitivity is 3-5 U/L. Single-test diagnosis isn’t recommended; it requires clinical-laboratory support.

Average testing time is 15-30 minutes, depending on reagent availability, preparation, and method (manual or automated).
